An Informal Language Intake Assessment is done to gather information on where your child is at in
numerous areas. This information allows me to plan and prepare custom-made lessons for
your child. The assessment will take approximately 60 to 90 minutes.

Note: This Informal Assessment does not take the place of Psycho Educational Assessment, nor
does it provide any type of diagnosis.

How to prepare for the Assessment:

Set up your workspace for success. Clear desk space and provide a chair that fits your
child. Please have…

  • 2 blank pieces of paper
  • 2 sheets of lined paper
  • Blue or Black pen (or pencil as long as led is dark enough)
  • Laptop or Ipad
  • Zoom – web conferencing software

Parent’s Role:

As I do not know your child, and I am not in the room with your child, I require a parent to be in
the room for the assessment and subsequent lessons until it is clear that your child doesn’t
need you to be there.

Interactions should largely be between myself and your child, however I will need your support
in the following situations…

  • Your child loses self-control.
  • Your child refuses to come to the computer.
  • Your child is distracted and unable to ignore the distraction.

The Language Assessment will include the following:

  • Spoken language
  • Phonological Awareness
  • Alphabet Knowledge
  • Reading letter and grapheme sounds
  • Reading decodable words
  • Reading sight words
  • Body position when writing
  • Penmanship & letter formation
  • Writing letters
  • Writing decodable words
  • Writing sight words
  • Passage reading fluency
  • Sequencing days of the week, months of the year, etc

Some of these skills are challenging to assess online. As such, I may need a parent to change
the angle of the camera to see properly. These skills are penmanship & letter formation and
body position when writing.

I will also need a parent to take clear pictures or scan the pages after your child has completed
the tasks and send them to me by email.
